Public Assistance (Mahajanadhara)

• Issued for people who has low income , helpless persons, sick, and disabled people.
• The application should be completed and submit to Grama Niladhari of the area. It should be presented to the social services division with the recommendation of the Grama Niladhari.
• If a person aged below 60 is applying because of a medical condition, a medical certificate is required.

Equipment Aid (Wheel chair/Spectacles/Crutches)

• Should be person having low income.
• Medical certificate to use the equipment.
• Grama Niladhari recommendation and relevant form

Scholarships

• These scholarships are offered to children currently attending school in needy families whose fathers have died.
• A letter stating that the student is being educated at the school where he is studying. (With the signature of the Principal)
• Copy of the birth certificate
• If the father is dead or separated, the death certificate or affidavit.

Aid for the Diseased

• Donations for diseases such as cancer, tuberculosis, thalassemia
• Medical Certification confirming the medical condition
• Completed form issued by the Divisional Secretariat
• A certificate stating as a Low incomer, from the Grama Niladari of the area.

Housing Assistance for the Differently abled

• Should complete the form given by the Divisional Secretariat
• Applicant should have a land (it should be confirmed by a deed)
• The deed of the land if there is no land and someone is willing to offer a land(This is done under the direction of the Divisional Secretary or the Social Services Officer).
• A letter from Grama Niladhari stating as a low income family.
• Certificate obtained from a reputable doctor regarding the disability condition.
• Housing plan and estimate obtained from technical officer.
• Birth Certificate or photocopy of the NIC for confirming the age.
• Documents or other documents stating how to obtain the excess amount for estimates above Rs. 250,000.

Self-Employment Assistance for Differently abled

• Should complete the form given by the Divisional Secretariat
• Medical report stating disability.
• Report of Social Service Officer.
• Details of the goods purchased if needed for self-employment .
• Self employment project report mentioned in the application form.
• Grama Niladhari report stating as low income earners.

Providing living allowance (LKR 5000) for Differently Abled

• Application
• Medical report obtained from a doctor on disability.
• Grama Niladhari report stating as low income earners/ Request letter.

Self Employment Aid for Single Parent

• This aid is given to the women, aged below 50 who is widowed or has abandoned by their husband. 
• Grama Niladhari report stating as low income earners.
• Death certificate of husband or affidavit stating being abandoned by the husband
• Priority is given if there is school children

Disaster Management Activities

• Donation
• Testing and recommendation for donations.
• Providing reports.

Vocational Training

• Directing differently abled persons to relevant vocational trainings
  • Age limit 16-35
  • Should be a person with special needs
  • Should be unmarried
  • Should have educational qualifications needed for the training
• Guiding for self employment

Construction and repair of commode toilets

• Completed application form

• Monthly income should be less thn Rs. 6000.00
• Applicant being a disabled person
• Possession of the right of construction site of the toilet by the disabled person. If not, the owner of the relevant place should sign an agreement with the Divisional Secretary, Social Services Officer and Grama Niladhari
