  1. Request Letter
  2. Completed Forms of Business Name Registration
  3. Copy of the deed where the business is conducted
  4. Land Reqistry[Pathiru] (about 2 years)
  5. If the business is conducted in alienated land
    1. permission grant letter by the owner
    2. the way he/she handed over the land to you
  6.  Receipt of money paid to the urban council
  7. Copy of Identity card
  8. Resident Certificate issed by Grama Niladhari
  9. Stamp worth of 10 rupees
  10. A photo of Business establishment certified by Grama Niladhari
